At Luna Integrative Care, we believe in transparency and your right to access your health information. In compliance with Texas Health & Safety Code Section 181.105 (HB 4224), we provide the following information about how to access your health records, contact our licensing authority, and file complaints if needed.

How to Request Your Health Records

You have the right to request a copy of your health care records from Luna Integrative Care. To request your records:

  • 1. Submit a written request via email to info@lunarecovery.com with the subject line "Health Records Request"
  • 2. Include the following information:
    • - Your full legal name
    • - Date of birth
    • - Date range of records requested
    • - Description of records needed (if specific)
    • - Preferred delivery method (secure email or mail)
    • - Contact phone number
  • 3. Identity verification: For your protection, we may contact you to verify your identity before releasing records
  • 4. Response time: We will respond to your request within 15 business days as required by Texas law

Reasonable fees may apply for copies of records as permitted by law. We will inform you of any fees before processing your request.

How to File a Complaint

If you have concerns about your care or believe your privacy rights have been violated, you have the right to file a complaint. We encourage you to reach out to us first so we can address your concerns directly.

Step 1: Contact Luna Integrative Care

Step 2: File with Texas HHSC (if needed)

If your concern is not resolved to your satisfaction, you may file a complaint with the Texas Health and Human Services Commission:
